The following Notice of Privacy Practices covers your protected health information and medical records with relation to independent medical practitioners.
Notice: RealWeigh is not a medical group. Any telemedicine consults obtained through this Website or Application are provided by independent medical practitioners including, but not limited to, RealWeigh Doctors MD LLC, an independent medical group with a network of United States based health care providers (the “Provider”). RealWeigh Doctors MD LLC (or your own medical provider if you do not use a RealWeigh Doctors MD LLC provider) is responsible for providing you with a Notice of Privacy Practices describing its collection and use of your health information, not RealWeigh. If you do not agree to be bound by those terms, you are not authorized to access or use our Website and Application, and you must promptly exit our Website and Application.

This Notice of Privacy Practices (the “Notice”) describes how RealWeigh Doctors MD LLC (“RealWeigh Doctors,” “we” or “our”) may use and disclose your protected health information to carry out treatment, payment or business operations and for other purposes that are permitted or required by law. “Protected health information” or “PHI” is information about you, including demographic information, that may identify you and that relates to your past, present or future physical health or condition, treatment or payment for health care services. This Notice also describes your rights to access and control your protected health information.

We will not retaliate against you for filing a complaint.
We must follow the duties and privacy practices described in this Notice. We will maintain the privacy of your protected health information and to notify affected individuals following a breach of unsecured protected health information.
