The following Notice of Privacy Practices covers your protected health information and medical records with relation to independent medical practitioners.
Please see Privacy Policy for a description of how RealWeigh Doctors MD LLC collects and uses Personal Data about you through the use of this Website, mobile applications, and through email, text, and other electronic communications between you and RealWeigh Medical LLC.
Notice: RealWeigh is not a medical group. Any telemedicine consults obtained through this Website or Application are provided by independent medical practitioners including, but not limited to, RealWeigh Doctors MD LLC, an independent medical group with a network of United States based health care providers (the “Provider”). RealWeigh Doctors MD LLC (or your own medical provider if you do not use a RealWeigh Doctors MD LLC provider) is responsible for providing you with a Notice of Privacy Practices describing its collection and use of your health information, not RealWeigh. If you do not agree to be bound by those terms, you are not authorized to access or use our Website and Application, and you must promptly exit our Website and Application.

If you are not satisfied with the manner in which a complaint is handled you may submit a formal complaint to the Department of Health and Human Services, Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/.
We will not retaliate against you for filing a complaint.
We must follow the duties and privacy practices described in this Notice. We will maintain the privacy of your protected health information and to notify affected individuals following a breach of unsecured protected health information.
